The Student Insurance Office cannot change a student’s address or personal information. To change your address or phone number:
- Go to the UI-Integrate Self Service site: https://apps.uillinois.edu/selfservice/
- Log in using your Enterprise ID and password
- Click on the appropriate campus
- Go to the 'Personal Information' tab
- Click “Personal Information”
- Click “Addresses and Phones”
- Select "Mailing Address"
- Update your Mailing Address information and Submit
Please note that your updated address will not immediately appear on Student Insurance Office online forms.
Once your information is updated in the UI-Integrate system, it will download to the Student Insurance claims processing program within two weeks.
